This package includes a comprehensive assessment of a youth's reading, writing and math achievement and a Sample Learning Plan (SLP.)  The SLP consists of a learner's background, overall strengths, areas of needs, and a list of essential and universal supports based on assessment results and history. The SLP is a ready-to-use summary of a learner's overall profile. It can expedite the planning and implementation of support at the school level.
